    The impact of life events on depression among elderly in Kelantan

    Background: Life events are believed to be associated with the development of depression. The extended years of living will invariably subject the elderly people to more life events. Thus, they are more vulnerable to suffer from depression. The aim of this study is to determine the prevalence of depression among the elderly residents in Kelantan and to elucidate the association between life events and the elderly depression. Methods: This is a cross sectional study, which was conducted in Kelantan from April 2016 until December 2017. Stratified random sampling method was used to recruit 422 elderly people aged 60 years and above. The Malay Geriatric Depression Scale-15 (M-GDS-15) was utilized to identify the presence of depression, while life events occurring in the past twelve months were evaluated using the Social Readjustment Rating Scale (SRRS). Data were analyzed using simple and multiple logistic regressions. Results: The prevalence of depression among elderly in Kelantan is 22%. Being older elders (aged more than 75) and staying in traditional house were found to have positive association with depression, while being married and performing exercise were identified as the protective factors against depression. The significant life events that contributed to elderly depression were death of spouse (95% CI =1.45, 26.34, p=0.014), changes of social activities (95%CI: 2.24,6.91, p<0.001), serious illness orinjuries (95% CI =1.15,7.84, p=0.025), changes of life habits (95%CI: 1.50,4.73, p=0.001), changes of sleeping habits (95% CI: 1.38,4.70,p=0.003), changes of lifestyle (95%CI: 1.32,4.61,p=0.005) and changes of numbers of family get-together (95% CI: 1.32,4.07,p=0.004). Conclusion: The prevalence of depression among elderly in Kelantan is high. The changes in social relationship, activities and lifestyles are significantly related with elderly depression. These life events are inevitable in elderly. Therefore, there is a need to detect early signs of depression especially in those who faced certain amounts of recent life events. Social interventions such as promoting healthy and active lifestyle among elderly also warrant attention

    Establishing a Sustainable Shariah-Compliant Hospital in Malaysia: Exploring the Pertinent Concept

    Malaysia has been a member of the Organization of Islamic Countries since 1969, and Islam is recognized as the federal religion in the country. The growing awareness of practicing Islam as a way of life among Muslims worldwide has significantly impacted the concept of hospitality services in the healthcare industry and tourism in Malaysia. As a result, there is an emergence of Shariah-compliant hospitals in the country to cater to the demand of Muslim consumers. However, the establishment of these hospitals lacks proper guidelines, leading to the need for developing standardized and regulated Islamic medical ethics and Shariah-compliant hospitals. This is crucial to ensure proper adherence to Islamic principles, especially in addressing concerns of the Muslim public and religious scholars regarding justified Islamic practices in medical treatment. To address this need, this study aimed to explore the elements or standards required in establishing Shariah-compliant hospitals (SCH). The study adopted a focus group discussion approach with 9 representatives selected through purposive sampling. Participants included hospital administrators, doctors, nurses, policymakers, academics, and representatives from non-governmental organizations (NGOs). Thematic analysis of the discussions revealed eight critical elements to consider when planning a sustainable Shariah-compliant hospital. These elements include ibadah-friendly infrastructure, Islamic medical ethics, Shariah compliance prescription, spiritual care support, Islamic medical service competencies, a conducive Islamic environment, affordability and accessibility, and an Islamic work culture. These findings serve as a foundation for future research aimed at developing a comprehensive framework for the establishment of standards for Shariah-compliant hospitals.     Most tertiary institutions offer entrepreneurial programmes and courses to the students. However, there appears to be a lot of challenges in maintaining the elements of business in the real world. This study highlights the obstacles in entrepreneurship among students from the Faculty of Plantation and Agrotechnology, Univeristi Teknologu MARA (UiTM) Pahang. The main objective is to explore the perceptions toward the obstacles in agro-preneurship among the respondents. The results pointed out that the students’ perceptions of lack of experience, finding the right partner, and lack of information are the main three obstacles in agro-preneurship. In addition, specific obstacles in agro-preneurship exist based on the student’s gender which is caused by different obstacles

    Component of sawt in hadeeth texts: Musiqi al-Quran in the art of tarannum

    The art of the reciting of the Quran is the concept of the musiqi al-Quran or the melody of the Quran in the Quranic reciting. It is associated with vocal artistry composing of two fundamental elements: tone and rhythm. The tarannum style of Quranic recitation is executed with vocal artistry that is simple, natural, and without the complement of any musical instrument; yet, the melodious recitation captivates and mesmerizes the listener. The objective of this qualitative study is to examine the vocal (sawt) elements of tarannum, using texts found in Hadeeth literature as a basis of the analysis. Data was collected through the analysis of documents, while data analysis was performed through the use of the ATLAS ti 8.0 software. The study finds that in the texts of the Hadeeth, a total of sixteen components of sawt al-Quran have been discussed: tarji’; qiraah layyinah’; husn al-sawt; ghaddan; husn al-tarannum; jahr; tazyin; husn al-Quran; husn al-qira’ah; huzn; tabaki; taghanni bi al-Quran; mizmar; tahsin; tahbir and talahun. The analysis in this study further leads to an implication that the absence of the aforementioned elements, affect a reader’s ability for tadabbur (contemplation) and khushu’ (concentration) in their Quranic recitation

    Assessing adherence to direct oral anticoagulants (DOACs) among patients with atrial fibrillation in tertiary-care referral centers in Malaysia

    INTRODUCTION Direct oral anticoagulants (DOACs), such as dabigatran and rivaroxaban, are now available for stroke prevention in patients with atrial fibrillation (AF) and are often clinically preferred over vitamin K antagonists (VKAs), such as warfarin. Data describing adherence to DOACs in real-life clinical practice in Malaysia are scarce. OBJECTIVES This study aimed to assess adherence to DOACs in patients with AF at Hospital Kuala Lumpur (HKL) and Hospital Serdang (HSDG), tertiary-care referral centers. METHODOLOGY This was a retrospective cohort study that included all AF patients who were treated with DOACs (dabigatran or rivaroxaban) in HKL and HSDG. Data were obtained from medical records and pharmacy database. Adherence was assessed by using proportion of days covered (PDC) over a 1-year duration and was calculated as the number of days that the medication was on hand over the total number of days in the time period. Good adherence was defined as PDC ≥80% RESULTS There were 281 patients who met the inclusion criteria, male 54.1% (n=152), with 75.1% (n=211) patients on dabigatran and others on rivaroxaban. The ethnic distribution was Chinese 49.8% (n=140), Malay 41.6% (n=117), and Indian 8.5% (n=24). Only 66.9% of patients achieved good adherence with PDC ≥ 80%. Adherence by institution was good in HKL with 85.2% (n=69/81) but poor in HSDG with 59.5% (n=119/200) [p<0.05]. CONCLUSION Overall adherence to DOACs was poor but it varied between institutions, potentially due to institution-specific administrative and clinical practice differences. Clinical care can potentially be optimized by identifying factors affecting adherence and implementing adherence-improving interventions

    A micropolar fluid model of blood flow through a tapered artery with a stenosis

    A nonlinear two-dimensional micropolar fluid model for blood flow in a tapered artery with a single stenosis is considered. This model takes into account blood rheology in which blood consists of microelements suspended in plasma. The classical Navier–Stokes theory is inadequate to describe the microrotations or particles' spin of such suspension in a viscous medium. The governing equations involving unsteady nonlinear partial differential equations are solved using a finite difference scheme. A quantitative analysis performed through numerical computation shows that the axial velocity profile and the flow rate decrease and the wall shear stress increases once the artery is narrower in the presence of the polar effect.
